The following medical forms are due prior to the start of the school year:

New Students:

• Preschool & JK - Health
• Kindergarten - Health, Vision, Dental
• Grade 1 - Health, Vision
• Grade 2 - Health, Vision, Dental
• Grades 3,4,5 - Health, Vision
• Grade 6 - Health, Vision, Dental
• Grade 7,8 - Health, Vision

Returning Students:

• Kindergarten - Health, Vision, Dental
• Grade 1 - Health
• Grade 2 - Dental
• Grade 6 - Health, Dental

ALL students in grades 3-8 that participate in a sport needs to have a physical (Health) form completed.
